fa شیوع عفونت‌های همزمان با ویروس نقص ایمنی انسان (HIV) در زندانیان مرد Prevalence of Human Immunodeficiency Virus (HIV) coinfections among male prisoners in Alborz province طب عفونی Infectious Medicine تحقيقي Original Articles زمینه و هدف: زندانیان در معرض خطر ابتلا به عفونت‌های خطرناک مسری از جمله ویروس نقص ایمنی انسان (HIV)، ویروس هپاتیتB (HBV)، ویروس هپاتیت C (HCV) و توبرکلوزیس (TB) قرار دارند. این مطالعه به منظور تعیین شیوع عفونت‌های همزمان با ویروس نقص ایمنی انسان (HIV) در زندانیان مرد استان البرز انجام شد. روش بررسی: این مطالعه توصیفی - تحلیلی روی 100 بیمار مرد زندانی مبتلا به HIV با میانگین سنی 7±39 سال انجام شد. از بیماران نمونه خلط و خون گرفته شد. برای بررسی هپاتیت B و C تست الایزا بر روی سرم بیماران انجام شد. پس از استخراج DNA این ویروس‌ها، بار ویروسی با استفاده از روش مولکولی Real time PCR اندازه‌گیری و سپس نمونه‌های مثبت تعیین توالی شدند و ژنوتیپ آنها تعیین گردید. برای بررسی عفونت توبرکلوزیس خلط بیماران به روش اسید فست رنگ‌آمیزی شد. یافته‌ها: فراوانی ابتلا به HBV، HCV و TB در بین 100 نفر از زندانیان مرد مبتلا به HIV به ترتیب 45 درصد، 3 درصد و 9 درصد تعیین شد. مبتلایان به HBV دارای ژنوتیپ D و ساب ژنوتیپ ayw2 بودند. 45 درصد بیماران آلوده به HCV بودند که از میان آنها 3 درصد آلودگی همزمان HIV-HCV-TB داشتند. از 45 بیمار مبتلا به HCV، تعداد 34 نفر (75.5%) ژنوتیپ 1a و 11 نفر (24.5%) دارای ژنوتیپ 3a بودند. تعداد 9 بیمار همزمان مبتلا به بیماری TB بودند. سابقه اعتیاد تزریقی شایع‌ترین راه (79%) ابتلا به HCV و HBV تعیین شد. بین متغیرهای سن، تعداد سلول‌های CD4 و راه انتقال با ابتلا به عفونت HCV در افراد مبتلا به HIV ارتباط آماری معنی‌داری یافت نشد. نتیجه‌گیری: این مطالعه نشان‌دهنده شیوع بالای هپاتیت C و بیماری سل در بین زندانیان مرد مبتلا به HIV است. Background and Objective: Prisoners are among the high risk population for contagious infections such as HIV (Human Immunodeficiency Virus), HBV (Hepatitis B Virus), HCV (Hepatitis C Virus), TB (Tuberculosis), and other dangerous diseases. In spite of other countries in the world, data about the prevalence and risk factors for infectious diseases among prisoners are spars in Iran. The aim of this study was to determine the prevalence of HBV, HCV and TB coinfections among male prisoners suffering from HIV. Methods: In this descriptive-analytical study, 100 HIV patients were selected. Microscopic examination (acid fast staining) was applied on sputum specimens and serological (HBsAg, anti- HCV) and molecular (Real time PCR and DNA sequencing) investigations carried out on patient’s sera. Results: Among 100 prisoners affected with HIV, the prevalence of HCV, HBV and TB infections were 45%, 3%, and 9%, respectively. All HBV positive cases were genotype D, subgenotype ayw2. Among the HCV positive subjects, 34 (75.5%) and 11 (24.5%) were genotype 1a and genotype 3a, respectively. There was no significant relation between age, CD4 and transmission route of HCV in male prisoners affected with HIV. Drug injection was the main route for the acquisition of HCV, HBV infections.
